A 29-year-old female presented complaining of a large, painless lump in her left upper eyelid that developed rapidly over several days and has been present for the past three weeks. Though painless now, she experienced moderate pain while the lesion developed. This growth occurred several times previously, though not to this extent, and it resolved quickly without any intervention, she said. The remainder of her examination was normal, except for biomicroscopically visible crusting around the eyelashes, blocked meibomian glands and a grossly visible hard focal lump in her left upper eyelid. This time, she purchased an over-the-counter product and used it as directed on the box without any relief. Based upon appearance and history, she developed a chalazion secondary to chronic blepharitis. There are several treatment options available for this patient, and virtually every doctor has a preferred approach to managing chalazia. In this months column, we examine the science behind each approach. Chalazion is a common inflammatory condition of the eyelid. They were seen at their local family care center and were given oral Bactrim (trimethoprim/sulfamethoxazole). The patient was diagnosed with a chalazion of the right lower eyelid. There was no recollection of prior trauma or insect bites to the area. A complete review of systems is negative except as in HPI. The parents first noticed it about two weeks prior. Shriver, MD, FACS August 23, 2014 The patient is a 4-year-old female patient who was referred to the pediatric ophthalmology and strabismus clinic at the University of Iowa for evaluation of a bump of the right lower lid. The patient had taken the medication for four days, but there was no improvement of the area. Normal growth and development and no history of illness. She was sent out with a plan for conservative management including warm compresses and topical erythromycin ointment. The patient was acting well and did not report any pain in the area. Bactrim (trimethoprim/sulfamethoxazole) suspension 10 ml, by mouth (PO), twice daily (BID) for 10 days, currently on day four. She was seen for follow-up 10 days later with continued enlargement of the lesion, so she was taken to the operating room for incision and drainage. Following the procedure, the chalazion appeared significantly improved (Figure 2) and it eventually resolved completely. Anyway that's a longwinded way of saying maybe it's not the chalazion surgery that's at fault here but rather the underlying problem that led to the surgery. Chalazion is a swelling in your eyelid caused by chronic inflammation of one of the small oil producing glands (meibomian glands) located in the upper and lower eyelids. This swelling is localized and often resembles a pea-like lump under your eyelid skin. A chalazion is usually a reaction to trapped oil secretions and not caused by bacteria, although the site can become secondarily infected by bacteria. When a chalazion is small and without symptoms, it may disappear without treatment. If the chalazion is large, it may cause blurred vision by distorting the shape of your eye. A chalazion is sometimes confused with a stye, which also appears as a lump in the eyelid. Chalazions usually occur farther from the edge of the eyelid than styes, and are usually not very tender.
